How does it work?

1. The employee registers independently, or the applicant/employer registers an employee by fully completing the form below.
2. The applicant/employer receives a confirmation email from MyDailyLifestyle to approve the invoicing.
3. After approval by the employer, the employee can start the program.
4. The employer submits a subsidy application via Mijn A+O.
5. MyDailyLifestyle sends the invoice to the employer.
6. The employer can submit the invoice to A+O for reimbursement.
